Phone add-on order.

Please enter your Account ID.

If you do not have your Account ID, feel free to email mac@empiretelecomnj.com for assistance.

Please enter the main account address. If you’d like equipment shipped to a different location, you’ll have the option to provide that later in the form.

Do you require a physical device or equipment from us today, or do you only need to order an extension/user?

This determines what the extension will be connected to, for example an app on your computer or phone, or an existing desk phone.

If you already have a phone, please select the phone model and provide the MAC address and serial number. If you don’t have this information right now, you may skip this question by clicking Next and provide it later to the project manager.

Please select the device you’d like to order. For photos and details on device types and options, visit empiretelecomnj.com/devices.

How many devices/users do you require?

Please provide the information for the new users you’d like to add:

• Extension: Enter the user’s extension or “Next Available.”
• Username: Display the name for the phone and directory.
• Email (optional): For voicemail-to-email and app login.

Please enter the exact address where you would like the device to be delivered. Make sure to include street, city, state, and ZIP code.
